There is only one fatality per character.

Believe it if you will, but I wrote Midway (WAY BEFORE we got the game) about Cage's tattoo (what it was, and yeah, I aws right about it being a dragon) and that's the same woman who told me it was. She really does work there--I can confirm that, if you believe me.
Oh, and IGN said that there were no pits or second fatalities.
